Consent and Release Form

The new school year's Consent and Release Form is now available. It is good for September 1st of 2009 through August 31st, 2010. Please go ahead and have it filled out notarized and turned in. This are necessary to have on file for any event that takes us offsite.

Golf Tournament Registration Forms

Registrations forms for your teams are available in the church office, the youth ministry office, and online. You can also pick them up in the sanctuary. Please fill out the form and turn it in with your team registration information and fee.

Student Life Info

Info for Student Life Camp 2009 has now been transferred over. Not the final payment is due March 29th. Almost each of you has used your account funds, please see me about the total you owe before paying. Also, you need a White Water Express Waiver turned in also for rafting trip at the end of the week, along with the Student life Waiver, and a Consent and Release Form.
